Carolina needs this form submitted either here, as an online form, or you can download a PDF, fill it in, and send it back to her, whichever is most convenient for you.

When she receives this completed form, she will confirm the date you've scheduled for your mediation.

Until she receives this COMPLETED form, your date is only considered to be "on hold."
